So yeah, this is my own form, not the best, but pretty decent please try it!

The file does not concern only enemy's they concern the file of any ninja rogue/missing or village active. Meaning that the ninja you feature in this from can be a ninja that has not betrayed their village(which it actually is intended for,but whatever).

Basic Info:
Name,age,village,gender,height

General Shinobi Info
Rank, Skills(based on a point system 1= basic, 2=average, 3= proficent, 4= advanced [5= caution, 6=extreme caution, 7= kill on sight, 8= Do not engage ,9= flee on sight, 10= inescapable opponent] - for overall skill

Personal Info

Important traits(diseases,states,conditions,whatever,etc.), History(short,need to know-ish info, age when they became a certain rank, really important stuff,etc)

Deaths

Death # at once(how many people they've skilled at one time) Cause of overall deaths(How many people they've killed in all)
