I remember standing on lego for fun, never hurt that much but I would assume the shock from not expecting it plays a bit role in the pain.

This comic can only be understood by people who have lived in a house that have legos and walk around barefoot. This happened to me a few times and ONLY late at night. It's hard to not scream when you're engulfed in incomparable pain.

however ... we don't know how hard and sensitive pony hooves are... so we don't know if legos work the same way on ponies or not

If we treat them like real equines, I would say, yes. Only the outside of the hoof is hard; the underside the hoof is very tender and easily can get bruised from stepping on hard objects. Horses can go lame, for a time, from stepping on even a small pebble, so I could see a lego brick being absolute agony, if stepped on just right.
