usually I really like your designs, but this one fall a bit short. still good, just not 'as' good.
the reason why is that she is too humanoid. the placement and proportions of the nose, eyes, jaw and the crown of snakes (hair) look too much like that of a normal person. were it not for the arms it could be some person in costume. I know you already went through too much trouble to change it, but in the future I advice changing the proportions of the face and other human-analogous features more.
put the eyes on the side of the skull, add multiple pairs of eyes, even odd numbers of eyes.
the crown of snakes or tentacles could run down the neck and body like a mane of a horse, or from all around the neck like that of a lion. multiple heads, odd number of arms, wings or legs, etc.

The thing is, the commissioner gave me a specific image of a medusa to Grimmify, and it was already pretty humanoid as it was. I added the classic Grimm features like the spikes on the back, and the skull faceplate. I even added horns, even though that wasn't in the reference picture that I was given.

I'm just saying that you could have de-humanize in addition to Grimm-ify if you catch my drift, since most Grimm tend to look rather un-humane. the only exeption is the nuke but it still has a weird mouth and it's proportions (for example the knees on the horse's legs aren't like those of a real horse in shape, it's 'waist' is impossibly thin, etc).
as a rule of thumb, Grimm should always look less human than the nuke. this is because Grimm, especially the newer ones, seeing how the story is turning a bit more serious, are meant to invoke the grotesque.
TL;DR: don't just add Grimm plates and glowy stripes, If you want to Grimm-ify something, you should deform it's proportions so that they invoke an uncanny feeling.
