The price any hero must pay to save someone else's life is to risk their own.My idea with this sequence (of which that art of Rei stopping a bus is part of) is some scene in some story where the two are doing something to save people's lives after some shit happens and a big fight is involved... and obviously the fight has injured her so greatly that it's pretty possible she's gonna collapse dead any second from the exertion alone.
