I'm not sure how that works with the hammer and barrel being off aligned. Care to explain?

Well, given that its shape is roughly similar to the Armi Chiappa "Rhino" and the famous Mateba "Unica" low-axis revolvers, I think I can help you out here, barring artistic license.
Typically, what you are looking at that appears to be a hammer on a low-axis revolver is in fact a cocking lever, not a hammer, sometimes called a "false hammer." The actual hammer is concealed entirely inside the weapon. The cocking lever is connected to the hammer via a mechanical linkage; pushing down on the spur like on a hammer moves the hammer backwards to prime it. On the Rhino, if you push it all the way backwards, it engages the double-action mode, and the cocking lever returns to the lowered position, though the weapon is primed to fire.
(The Mateba Unica weapons used a much more complicated system that earned the distinction of being actual semi-automatic revolvers, with a moving frame to the rear of the weapon that functions very much like the slide action of a traditional semi-automatic handgun. The main difference being that the action didn't eject a shell and feed a new round from a magazine but instead rotated the cylinder and primed the hammer. I don't think that's what is going on here; I think this weapon would function much more similarly to the Rhino.)
