When I made this, I think I still used UCCW..

These days I just use stock transparent since iv become lazy

Thanks! I actually decided to use the red one on bouth screens, just blurred on lock...

In photoshop make new file 1080x1920 px, make rounded square (press "U" for the vector tool and select rounded square, you can single click anywhere on the picture and it will ask you what size shape you want to create, create 1080x1920, I used around 30px rounding), add the wallpaper you want as a clipping mask on this shape, create black filling on the background and there you have it! Save as JPG and move it to your phone! 

I use QuickPic to setup my wallpapers on my phone.

There is also app in Play Store that allows to create this play.google.com/store/apps/det… But i prefer to make them my self..
