Skill, practice, patience, and lots and lots of time?
Tattooing the old way isn't easy by any stretch of the imagination, but it's not impossible either. Heck, I saw a news story about a guy in Hawaii who does traditional tattoos the 'old' way, and his work is phenomenal.
