This is funny but it so true that's its annoying that they ALWAYS say your last name, I think what they should've done is list a VERY long list of names that they have recorded and then when you pick one THEY SAY FOR *!?#%# NAME!!!

Considering that Shepard is military it's only customary to refer to him/her by his/her last name. You usually do that in the military.

Ok, I agree. Shepard is a Alliance military commander, but consider this -Garrus, Tali, Wrex or Liara aren't the part of Alliance forces so they have no real reason to call him like that. Especially Tali, Liara or Garrus when you get really close to them.

Garrus was a C-sec officer. Same thing there I suppose.
Tali and Liara might do it out of respect, or perhaps because everyone else do it.
When you always present yourself with your last name only then you should expect people to refer to you by your last name only too.
