So when we're talking 'blunt and tells the truth' are we talking a guy that is a jerk all the time (except with children of course) and only ever says negative things under the guise of being "honest", or will he bluntly and aggressively tell the truth when something positive is at hand too? Is he like this even with the expectant in a situation that requires triage?

He doesn't talk much in general, but when he does it's either sarcastic or the plain truth! I think the situation will depend on whether or not he'll proactively tell the truth - if it's not necessary, most likely he will just keep quiet.
I believe as a doctor, he'd be pretty curt and professional. He can be empathetic, but tends to be emotionally distant because he's not very good at it. He relies on the nurses to do most the emotional support, but if it comes to it, I don't think he'd be a complete asshole to someone that is actively dying.

I'm kind of imagining now that he'd break his silence to aggressively tell positive truths if someone was questioning the capabilities of children or his sister in that case. They seem interesting.
