oh gosh her torso looks so painful! I don't think the ribcage and pelvis can be at such extreme opposites without the spine breaking...

Actually, yeah; they can. It depends on how flexible the person is. Your average woman should be able to do this with no pain, and little discomfort. In an action situation, the pose is held for such little time, it shouldn't even be enough long enough to register in your mind.
If a woman is fairly stiff, I could see this being fairly uncomfortable, though.
