The infection process is brutal:

A Combat Form is not a natural organism, but rather a mutated host infected by parasitic Flood Super Cells implanted by an Infection Form. Successful creation of a Combat Form requires a host with sufficient biomass and calcium store, and an infection form to infect the host. The flood don't kill their victims, they burrow into their chest and attach to their central nervous system. The flood combat forms are actually more of a vehicle then a creature, as you can't kill the body, only the infection form inside.

Not necessarily as all control is lost. The gravemind (the hive mind that controls the flood) absorbs all the knowledge experience and memories of it's victim. So basically, as said, they use the host as a vehicle. It's not even it's own living thing anymore.

Yeah, they can see the mutations and they'll slowly start to lose their memories as all this is happening. Eventually they are pretty much life less husks. And if the infection form is old (Like the one that infected Jenkins) it's possible you'll still think and be able to see what's going on with no control over it.

let me explain to you the horrifying infection process of becoming a combat form (most flood are combat forms, then there are carrier, and infection forms)

The infection form penetrates your skin with it's tentacles. It then burrows itself into you and attaches it's tentacles to your spine. It takes control of your central nervous system making you unable to control your arms or legs. It liquifies your organs and uses their nutrients to grow out tentacles.

It pushes your head out of the way to make room for itself.
It send your memories and thoughts to the gravemind, after which they are no longer available to you.

You start to mutate, forming tentacles, your appearance is changed based on the needs of the infection form.

And you are awake and alive through all of this. Sometimes the host survives even after becoming a combat form fully.

Pretty much I just own the whole series, read the wiki, read the books, read the comments on the developer's official website on the subject.
I'm just a huge fan of halo.

Basically you need to know this:

-It climbs and burrows into your chest
-It attaches to your spine, controlling your central nervous system and thus rendering your arms and legs useless
-It turns your internal organs into a liquid it forms the tentacles out of
-You can survive through the entire mutation because the infection form keeps you alive
-It mutates your appearance
-All of your memories are slowly transferred to the gravemind
-You can't do anything to stop it, you can only "watch" your actions
