I've gotten a few comments on Sapphire's singular eye, or lack thereof. This is just my take on it.

Based on other times in the show, the more in-sync a fusion is, the more humanoid they are. The dynamic of the relationship, I think, determines the overall appearance rather than how many physical features the ones fusing have. Stevonnie is incredibly in-sync and has pretty much no abnormalities because Steven's and Connie's relationship is very pure and trusting. When you get into Opal, Sugilite, Malachite, etc. the dissent and disagreements come out in physical form. Opal obviously has two eyes and four arms, but Pearl evidently had a greater connection with Rose Quartz, and yet Rainbow Quartz still has four eyes. It depends on the fusion.

Overall I'm not sure what Sapphire actually looks like, maybe we'll never know. Personally I'm not sure it would made a huge impact, Ruby and Sapphire are obviously pretty tight so I wouldn't be surprised if an extra eye is the only extra feature Garnet has.

Then again, Alexandrite is basically Sugilite + Pearl, and we have no idea how many eyes Alexandrite has. She does have a giant monster mouth though, has six arms, and is huge. I think gem relationships manifest in different ways, we can only hope the show offers more insight into this.
