I've always liked this shape to sidearms.
If the barrel would line up with the lowest cylinder, I'd expect this thing to be even more controllable. A veritable marksman's revolver.

I think it would, to be honest. Some of the sound would come from the wheel, but the suppressor would lower the sound of the bullet and the gases leaving the barrel. :L

Strify. A suppressor doesn't "suppress" a sound, as in, erasing the sound. The revolver wouldn't be "somewhat" quieter than a gunshot, it would be much quieter than one.

It would not be very quiet at all with that amount of gas escaping the cylinder gap compared to a well designed supressor on a sealed gas firearm.
That is my point.
