It really is a long process. There are several layers of shading and highlights and such.
I used some low opacity brushes to build up the look.

Well those are definetly among the best handmade realistic mountains I have seen , comparable to the ones I saw made by Tom Patterson on shaded relief .

The outlines of this and the Middle Earth map look not-even-that-much-alike.  I noticed that too but the similarity is so vast that it is almost not there  
It probably just happens if you stared at that map for hours and days looking at the once referenced places and Frodo's way, remembering every event that took place along his path ^^
