Karmic Retribution is only a thing because Frisk is feeling remorse for her killings (I think, In not sure on that) and takes the poison damage however we are not talking about Frisk we are talking about Blanc a goddess who is know for killing monsters on a daily bases and because Blanc would not feel remorse about killing any of the underground monsters so would not take the poison damage.

it's not frisk u play as when you get to sans, sans is then fighting chara. Karmic Retribution measures the leval and how many innocents you've killed WITHOUT remorse
