It's actually both, but the manga's only in Japanese. <_<;
So I've only seen the show, which is amazing. All episodes can be found on google video. (Just search Elfen Lied 1, Elfen Lied 2, etc.)
But be warned, the show is very bloody and has several scenes of nudity.
