I do admit that it's a fantastic use of symmetry and dramatic lighting, and a very original piece of artistic nude. Although I must say, that kind of symmetry is physically impossible for a human being, so shouldn't this be in Photomanip?
I won't make any assumptions about that model here, but those pelvic bones cannot be even close to natural. Even very thin people I know don't have their pelvic bones curving outward. 
Apologies but, this doesn't appeal to me because of the disturbing pelvic bones. Otherwise, brilliant image.

It's the lighting used to create that effect and the fall of shadows. Because her legs are extended out and knees pushed up it creates a sharp line to the pelvic bone and inside of legs cartilege stand out. She is sucking in her tummy as well as being on her back which will make her stomach look hollow and good lighting will make those shadows darker and give her a unusual shape with fall of light.
No, it is not a photo manipulation. Just really really creative use of light and shadow with the knowledge of the human body and how to distort it.

One of the most different pics I've ever saw. Beauty is in the eye of the beholder, everyone keeps saying for the model to eat, but...everyone knows when someone lays on their back, they look much thinner in a frontal shot. I love this shot. Thanks for sharing.
