You have to feel for Ratchet in that situation, given that his oath and position is practically identical to our medics. Even if not all swear it in, it's still "above all else, do no harm."

Yeah, that's what attracted me to write it, he's a medic, yet he's fighting a war, can't get more conflicted than that.
As for the pursuit by TC, there's actually a bit of contradicton in the comic. In #0 TC is pursuing them and Ratchet swoops in, then in #3 he recounts how he saved them and it shows him saving them from Runabout and Runamuck...
So, er, I kinda implied that there was a sort of connective bit (unseen) where Runabout/muck are joining the fray just as TC has had his wings clipped (hence the vapour trail in panel 4). It's not tidy but it does try to tie things up a bit and delve into his psyche a little.
