I gave a party of lv 1 newbies the Holy Hand Grenade of Antioch (20d20 radiant in a 20 ft sphere) as a reward for fighting the Judean People's Front (Roman campaign) and the one that got it rolled 3 Nat 1s that session, it was meant as a way to let them escape situations where I was heavy handed without me fudging dice (also rule of cool) but he was paranoid that he would roll a Nat 1 while trying to use it so he still carries it!

There was a swamp filled with dragon dungs (poops).
A friend of mine (fighter) and I (thief) argued because a dragon dung might sell nicely to any "science" community.
But he didn't want to smell bad, so I took them, filled it inside some containers.
Then our DM decided to be a butt, the dragon appeared just after. And it's venomous.
Since I play Monster Hunter, and I got dungs, I decided to throw one to the dragon, thinking it might make it go away because of the smell.
But then..... I rolled one.
So.... Fighter's face + dragon poop = unconscious for the rest of the day. (obviously we all died.)
(which is kinda bad luck because the day before, he decided to intimidate, yes, intimidate 10 drunk people on the street, rolled one, got bullied instead, wasted half the day away in a clinic.)

I had a guy in Vietnam freeze up with a live one. He pulled the ring but could not throw it. Massive panic attack. We tried to get him to let us put the pin back in but he would flail about screaming clutching the now armed bomb in both hands. Now all of this was while our firebase was under attack. We left him clutching that grenade and finished business.
It took the doc giving him a knock out injection to get that dang grenade from him.
Had one in training toss it up a slight hill towards a machine gun position. He short tossed it. The round ones roll nicely. Rolled right back to him. I just stared at him!
