You could ask the same question about nearly every other pony. Sweetie Belle? No idea. Shining Armor? Who can say?
As for Spike it's pretty obvious. Adopted kids. There was a whole episode about it. Plus there's the fact that he's a different species, age, and gender from most of the ponies he spends time with.

It's on and off, Shining Armor's the big brother who moved out/was away most of your younger years that you remember fondly, etc. It's a big list. Spike's just growing up. I thought Scootaloo was also the adopted kid, as Rainbow Dash is the only family she has until R.D's parents take her in(or implied did), while Applejack and Applebloom were to be orphans.

I really don't get why this is such a big deal.
Spike is dragon, one of the most diverse, mystical and a member of a race that ponies clearly know so little about. And as a part of his sub species growing up, he develops wings for flight, meanwhile Scootaloo is a Pegasus with a clear disability.
