Original Vector bySurprise is a 1187 year old Pegasus pony, granted immortality by the Paragon of Order in exchange for her unending service to him. Surprise is playful, childlike, and is always wearing a smile on her face. Despite this, she is incredibly intelligent, violent, sadistic, and makes everypony around her incredibly uncomfortable. Surprise is very manipulative, and has the unique ability to hold others hostage to her will through her own personality.
Surprise is generally very loud and obnoxious, yet has a natural aptitude to stealth and espionage. She loves nothing more than appearing from a hiding place and shouting her own name in order to scare those unlucky enough to be near her. She is not above hurting other ponies for her own amusement, she sees the world and it's inhabitants as her playthings, and is not somepony to be taken lightly.
Surprise is completely insane, and when backed into a corner, she will fight to the death with little regard to herself or any possible bystanders.

Which means she is good at being a villain.

Actually, now, I'd rephrase it into something like this:
"was her illness induced by some nefarious doing from somepony or someone else?"
Then, I'd realize I could find answer in your fanfic series.
