How to Apply :
-Apex Launcher:
Apex settings > Theme settings > Eleanor Icons > Apply ( Apply icons only )
-Go Launcher EX:
Themes > Installed > Eleanor Icons > Apply
-Nova Launcher:
Nova Settings > Look and feel > Icon Theme > Eleanor Icons
-Holo Launcher:
Appearance Settings > Icon pack > Eleanor Icons

It may not sometimes. Just long-press on it and replace with the phone icon from the icon pack list.
