Description The LNS Constellation, CG-5.
There are five ships in this class, as testbed ships under the Royal Lucanian navy in 2013. They are sixty year old ships, modified over the years. They are the second oldest ships commissioned in the Lucanian navy, the exception being the wooden masted 28-gun frigate LNS Ronkonkoma, built in 1771.

The Constellation class vessels carry a powerful AVALON2 weapons system, which is derived on the potent AVALON national defense network which tracks all air, sea and land installations/traffic. Two Phalanx CIWS, two Mark 45 guns, and two Mk26 missile launchers are standard with this vessel.
Two ships of these class(CG-9 and CG-10) are outfitted with the Mk10 experimental railgun.

These ships would see a large amount of action during the 2013 Luco-Theolian war.

Correction: CG should be DDG. CG stands for Cruiser, the Constellation class is a destroyer(DDG)

uh, I think the SM-2 launchers, and the AEGIS are a bit heavy for these relatively small ships.

You have a point with the launchers, but the AEGIS shouldn't be too heavy...

Depends on the version, that's true. However, AEGIS needs a lot of power. If you have exchanged the engines for more modern ones, you should also change the funnels (ie. make them more gas turbine-looking).
Personally, I think that modernizing old hulls is quite viable to a certain degree if you do not have anything else, cannot build or buy more modern designs or if you want to relegate these ships to other duties (training or coast guard).
Keeping 60 year old hulls in service and modernize them this heavily must either mean you are desperate (fighting a war against a superior enemy) or you have so much money, yard-capacity and manpower that you do not care

Good idea with the funnels.
The country described, Lucana, is indeed fighting a war against a numerically superior enemy, as demonstrated in this map:

The Lucanian military is outmatched 7:1, but can match their enemy in technological superiority.
As you said, these ships were brought out of mothballs because of the desperate situation.
