The gray background is to make the image easier to see. You should delete the layer before printing.This will work best against white elements.
I hope that someone with 3D modeling skills would model Aya's helmet, Razer's headpiece and Saint Walker's head, and either share them (on Thingiverse etc) or put them on Shapeways.

You may need to rasterize the texture before you apply it.

I assume photoshop or gimp would be able to do it.
In photoshop you can place the pdf as a smart object and can scale the object with the transform tool.
You might be able to just import the PDF straight into Blender, it is probably worth a shot.
The PDF is actually vector, so it should be infinitely scalable. You probably want it to be rasterized at the size you want the texture to be, instead of scaling up a small texture on your model.
The PDF is very small, because it isn't meant to be printed very large.
