Yeah, it does help to be careful, but unless you use a lot of lubrication it's pretty much fruitless (unless you're some sort of masochist). The anus isn't built for sex, it can't stretch nor contract like a vagina can. No matter the size being inserted, your butt's going to hurt! (or at least feel a little awkward).

Not really, though many kinds of hatred are caused by fear.
On the other hand "phobia" does mean "fear" as well as "hatred", depending on the context. This theme had already come up, but there's the wictionary (certainly among others) to fortify this.
It's also used as "repelling" or to state some kind of aversion, like in "hydrophobicity" in natural sciences.

Well, as mentioned by the other deviants, a phobia is an irrational fear, meaning that there is no reason to be afraid of something. Even though most of the time the fear could be rational (like hights or death), the irrational fears are usually the ones that makes it, well, irrational. Like me, for example: I hate bees -- well, I used to hate them, but now I just try not to get in their way -- and I've been stung by them, am not allergic to them, yet I still have this feeling of dread they're going to sting me 
Then you have the rational part: survival of the fittest -- or as my mom would call it, survival of the bullies. It's along the lines of: "If I don't beat this chump into the dust right now, he'll do it to me instead and I'll be the loser of this outfit." It's stupid reasoning, I know, but most reasons like that are based on fear, most of time rational -- to them.
So, at one part, the fear is rational, while objectively, it's not. Plus, homophobia (fear of gays) is more akin to xenophobia (fear of foreigners or aliens), meaning they're afraid of a person wit a certain label that they find negative, be it religion or personal discomforts with life. The part where the homophobic men are afraid of being treated like crap like they do to their wives, that's along the lines of a very insecure bully.
I just see this as a double-sided meaning with the base terminology of rational and irrational fear; it could be true, and it could not, especially when two very opposite things are true in the whole context.
