Ahh I'm probably wrong then. I read that the mangaka said that

"Grell is male anatomy wise. The reason his gender is so confusing is because Grell wants to be female, therefore refers to himself as such. But Grell is definitely a man. Hence the no boobs. Now the question of whether he's gay or not depends on whether you treat him as a man based on his anatomy, or if you treat him as a woman based on his personal preference. Either way, he has a thing for Sebastian, which is why he hates Ciel so much."
