2021 UPDATE: VIEW THE TUTORIAL/GUIDE HERE
With this tool, you can:
ADD vertices with faces
vertices on their own (without faces)
faces on their own (need vertices already placed)
new material layers
chain of bones
unconnected bones
bodies with joints
bodies without joints
joints on their own
CHOOSE the types of bones you place
the type of bodies you place (static, dynamic, bone-matching)
the shape of bodies you place (sphere, cube, oval)
the group of bodies you place
CHANGE the types of bones already placed
the parent/child of bones already placed (what bone they follow, what bone they point to)
the type of bodies already placed (static, dynamic, bone-matching)
the shape of bodies already placed (sphere, cube, oval)
the group of bodies already placed (from group 1 to group 16)
the bones that bodies are attached to
the bodies that the joints connect together (setting/changing bodyA and bodyB)
Using the knife tool:
At the end there's a labelled diagram of each tab so that if you need a quick reminder, you can skip using the buttons on frame 2.
If someone's already done a guide on this I swear I'll flip my desk so hard

i got a problem. Cant create the vertices with faces even if the option for that is selected... What do i do now????

Go to advanced editing (a tab next to Move/masking) and click "New". Doesn't have anything to do with the tool, but that should also do it

Okay, thanks. How do you make the newly added material appear (I tried and I didn't see the texture I added).

You need to add a UV Decal (And of course, have vertices on that material). Select the vertices you want your texture to be on, then go to edit>vertex>create UV Decal. make sure to look/have the camera look right onto the vertex you just made!

Could you possibly help me a little further by expounding a little more about adding faces? Once you have the "I put surf" button selected, and click the Start button, what do you do next to add faces to existing vertices? Do you have to select anything further to get it to do its thing?

Do you mean the box for advanced editing? If that's what you mean, I found it in my version in the PMD View screen by clicking the "Adv" button on the top right area of the screen. I'm not sure if that's what you meant to ask tho.

Oh it was wonderful!
I actually found out some really neat things after playing with this function.
I learned how to make bones, that are attached to IK. The orange bones in the knees, ankels, and miku's hair and tie.
Yeah I finally figured out how you do that!
