Well you have to be core to use it in some places, but all members can use it in journals. Simply scroll down the right side of the page and copy the thumb code. Where you paste it will show the sign :3

Your first question:
You put it under things like art status on either your profile or jorunal etc.
To do that you scroll down and copy the thumb code on the right side of the screen, wherever you paste it (profile/journal) will show the sign.
Your second question:
I just used photoshop and made it from scratch. There were no short cuts or special things I used to make this.

it does, and its quite expensive actually. :/
It is a very good program but its usually way over the price range of most people. There is a free trial though.
