Since she's been around him since his infancy, I can almost guarantee you that she probably has. It also helps that ponies largely don't seem to have the same senses about modesty like we humans would. Displays of such...physical traits...doesn't seem to be as taboo in Equestria.
It's actually kind of fun to think about, really. I'd bet all of those fan pics of ponies in more revealing poses that certain fans seem to love so much a pony would look at and probably go "yeah...so what?" because socially speaking, it just wouldn't have the same sort of impact for them.
Anyway, back to the issue of Spike and what Twilight has or has not seen, it occurs to me that in this instance with communal bathing, it wouldn't matter so much because Spike's "maleness" would still be kept hidden. Thus it's all on Twilight and for Twilight, she really doesn't have anything Spike wouldn't have seen before (and with all that bubble bath, it still wouldn't be especially visible), so with that in mind, Twi's discomfort over it must come from other reasons.
There's the occasional indication that ponies may view bathing as a semi-private affair (otherwise, why would they have shower curtains?) but we've also seen instances of group bathing with them without any hullabaloo over it too, so I wouldn't be satisfied with that for an answer. Maybe Twilight just likes bathing alone, and indeed, seems to have thought she WAS alone. Or maybe she draws a line on helping Spike wash his back like he requested.
Or maybe I'm just reading too much into what's supposed to be a simple comic.
