So Spike can't "wander" around. He also can't see Equestria rebuild. So it appears he's not dying as I thought from the previous page...he's leaving.
Now the question is why. Why is he leaving?
Okay...theory time!
1. The Good Ending: Twilight is alive and well, but they are both leaving Equestria to find true healing and peace, like how Frodo went to the Grey Havens.
2. The Neutral Ending: Twilight is not dead but vanished, and Spike is going to find her, leaving her fate ambiguous.
3. The Bad Ending: Twilight died and Spike has no place in Equestria anymore.
What do you all think? Since the comic seems to be ending on a hopeful note in contrast to what came before, the bad ending seems less likely than before. However, I won't place any bets just yet.

We still don't know if he's talking about leaving.
He may be referring to being unable to forgive himself for falsly accusing Twilight of betrayal when she was replaced and imprisoned this whole time. Probably...

That's another possibility. But how would that tie into him not having a chance to see Equestria rebuild? If he's referring to his own rebuilding, then...wait...I think I get what you're saying.
Twilight died, and Spike won't be able to mend his relationship with her because they didn't get to say goodbye and he never got to say sorry?
If that's the case, then that'll fall under the "Bad Ending." Actually, scratch that, that'll fall under the "Big-Fat-WTF-Bummer-Dude Ending."

He mentions that wounds will heal in time, as ponies strive to get better. He then says he won't be given such a chance.
He's pretty much saying that his own wounds won't be given a chance heal in time. The "wounds" may be his broken relationship with Twilight and how he failed her. An imposter pretended to be her without him knowing for 6 years. And when a Zygon was revealed, he believed that Twilight had really joined the enemy.
He knew Twilight for a thousand years, and quickly abandoned her when she changed. Not once did he suspect that she had been replaced.

I see what you mean. So...he's gonna live with that guilt? I can see that, as sad as it is.
So I can see some other outcomes now.
One: Twilight is gone and Spike has no chance to make things right with her.
Two: Twilight is alive but won't forgive Spike for his mistake. This is probably the least likely one given that Twilight still seemed to have a compassionate streak even after emerging from the cocoon.
Three: Twilight is alive and forgives Spike, but he'll struggle with the guilt regardless. Nevertheless, Twilight will be there to support him. As Spike was there to comfort Twilight in her darkest days, now the tables will turn and Twilight will comfort Spike as he wrestles with his failure.
That's what I suspect, but I must be prepared for an unexpected turn! What do you think?

I'd go for #3. But if the prophecy is true, Twilight may not recover from that final battle...
But yeah, be ready for unexpected turns. Even I'm not sure how these final pages will play out, and chances are that my theory may be way off...

Well, I've hypothesized that the prophecy might be a Dark Prognosticus from Super Paper Mario in that it was wrong all along, or at least averted in some way. Another possibility is that the prophecy was misread. I mean, it literally said "Twilight would not outlive her friends." In the context of this story, that could mean a number of things:
1. Twilight's friends will still live in her heart and memory.
2. Twilight's friends will live in another time parallel to her own.

4. The Silly Ending: Twilight's alive but thinks she is a bird so she is migrating south for the winter. And Spike is going to follow her to figure out where she'll migrate to each year
