I don;t really understand how this creates the background with hearts, wouldn't it just be a heart shaped picture?

HI sorry for the late reply!
It's a bit difficult to explain in a simple way, but I can try:
First of all, it can't make a heart-shaped image because the cut-out is far too close to focus on, as a result it blurs the card completely and slightly distorts the image depending on the size of the cut-out.
Now for the lights: When light enters the lens it shapes according to the aperture shape(which is generally circular or polygonal - this causes any unfocused light in the shot to shape accordingly. That's how you get the pretty little round lights in shots like these: [link]
So by making your own aperture in the shape of a heart, you're reshaping the light that enters the lens into a heart-shape. Does that make a little more sense?
Try holding a heart-shaped cut-out in front of a torch in a dark room near a wall, the light would be heart-shaped. And that's very similar to how this works if you imagine the wall to be your camera's sensor and the torch the incoming light source. The sensor picks up the light in a heart-shape and relays it as so in the final image.
It might not be 100% perfect or correct, but I hope it helps!
