The cheerful way Medic said "Tumors!" was my favorite part of Expiration Date. I would pay to hear Tara Strong say it like that in Twilight's voice.

....You know, I know this is a mlp reenactment of Expiration Date, but I still expected Twi to be at least a tad smarter than Medic. Tumors on bread makes no sense. Not even in the the TF2 universe, ad there's some pretty bizarre crap that goes on in that universe.

Smart as Twiligt is, she's not a font of limitless intelligence.
In the TF2 universe, ALL things make sense (I mean, Soldier's roommate was a magician). Twilight's learned, but she's always learning more.
The thing I love about Twimedic is that both characters share important traits, including the ditzy genius trope; that is, being quite smart in an intellectual manner, but often lacking in common sense. To be honest, for this reason, Twimedic is probably my favorite of the changes I made.
Just imagine her shaking the tube saying 'It hates me so much!'

Not really.
While Applejack has a southern accent (not the same one as Engineer, so you know) and the southern themed items fit her, she doesn't quite fit other elements of the character too well; particularly the smart, scientific, and squishy ones. The Engineer is almost completely reliant on his buildings, and unless he has the gunslinger, he spends most of his time sitting around.
Applejack, in my mind, is a doer; she goes out and gets her hooves dirty. She's also tough and versatile, able to put up a good defense or offense when nesessary. Pinkie, meanwhile, is the closest thing we get to a gadgeteer in the Mane 6 - her helicopter bike, party cannon, welcome wagon, and various party accessories make her a great match for the Engineer in my mind.
Bonus points that Engi seems to have the greatest connection to Australium of the group; AKA 'Miracle Gravel'. Demoman also has a greater family history, and his favorite drink is scrumpy (made from apples) (also made him a good candidate for Rainbow Dash).

Yes, actually. She was a close second for the role of Medic, and indeed would have filled it if I had made Twilight Sparkle the Engineer; Mane 6 took priority, so Zecora sort of lost out on that one.

I'm building off your suggestion for a background pony team, and Octavia, Time-Turner, and Zecora are all vying for the role of Medic!

If they all must be in there, I would give Medic to Zecora as the closest thing to a doctor among them. Time Turner might be a Doctor Who expy to many, but so far canon most seems to recognize him as a local clock-setter, so you could get away with making him Engineer. Octavia might work well as the Spy due to her sophisticated air.
