Its the spirit of the Dark Crown. When Twilight witnessed and wasΒ drained by the Dark Crown, she was "marked" by its spirit which would compel her to bring forth the Dark Crown for Red Harvest in her greatest moments of weakness, all while she was completely unaware of it. As a side-effect, such victims will often see hallucinations of this dark ghost stalking them, tormenting them.

Is this the effect of the crown when she was first exposed to it accidentally as a filly where it had nearly killed her if not for Celestias' intervention, and what we're seeing is a fragment of Blood Harvests' essence?

The crown got Starflare, then it went after Twilight...apparently it didn't affect her immediately. It DID affect her around the time she faked her death up to when Harvest explained it to her....basically Starflare and Twilight are what this story is all about, and hence BOTH of them are the important ones in this story.
