You really seem to have captured the essence of doubt here, the girl being obviously in need of help, yet hesistant to grasp the hand that wants to pull her out and help her.

Thank you very much - I am glad that you found it!
I use Photoshop (CS4 to do that one). Lots and lots of layers, and lots and lots of blending and masking and texturing, and then I end up basically painting on it, like in the case of her hair -- a lot of that is just hand painted. I have a tablet that I use. I hope this answers your question!
