Starfleet, and, by extent, the Federation was the dominant force in the Galaxy for more than a century. The Federation gre in size and power, and Starfleet was both an enabler and a protector of this process. But the Galaxy is a harsh, unforgiving world, and there were things out there that were beyond even the wildest dreams of Humans, Vulcans or anyone. Starfleet, mighty as it be, was humbled during the encounters of the 2260's, such as the "Planet killer" event that showed that even the powerful Constitution class was but an insignificantr dot on the galactic maps.

The wider universe is ancient, and some of the oldest players still exist in one form or another. Humankind and their friends and allies are latecomers on the stage, which is still littered with the toys and props of previous eras' dramas.
Sometimes the past isn't really past. Sometimes we get to have a bad day originally meant for someone else entirely.

Indeed, you got it right. I actually wrote a little fic/essay on this very thing:
The State of the Trekverse, year 2265The Universe is old. Much older than current scientific estimates point out, though its current cycle begun indeed some fourteen billion years ago. It has been ravaged by countless temporal disasters, causality incidents, space-time folds and the like. Most of these are of natural origin, yet many of them were feats of science gone wrong (or actually right, depending on the goal). The former type has larger effects, creating ripples throughout space-time, leading to the eventual collapse of the current incarnation of the Universe. The latter ones have localized effects, re-writing space-time in the few nearby galaxies from their points of origin.
Life is abundant in countless billion trillion forms. Some originate from the previous universal cycle. These entities are old, powerful and god-like, yet tired and don't usually meddle with anything other than their own lives. Roddenberry himself said they're father and son. And Kirk lobbied a lot for Decker Jr. to be the next Captain of the Enterprise after the Five Year Mission. This is why Decker interpreted Kirk's takeover as a personel betrayal

Ah, yes. One of the best damned episodes of TOS, and probably the franchise. Poor Matt. I always headcanoned that, due to his logs and how Jim Kirk initially treated him and his reaction to Matt's suicide run, that Commodore Matt Decker was a damned fine officer and one of the 'saner' flag officers in Starfleet.

It's fantastic. I can see the uncaring behemoth targeting the Constellation after their first attempt to engage it does nothing to the machine, and then the slow build-up of energy as Commodore Decker & crew realise that there is nothing they can do, they're dead in the water.
After the damage is done Decker sees it's gaping maw get closer and closer in the static-filled viewscreen until it passes by without a second glance. The cold, uncaring, machine logic of the alien vessel being the straw that breaks Decker's psyche.

The U.S.S. Constitution, captained by commodore Matthew decker is now just a floating hulk, after investigating and discovering the giant alien machine that destroyed an entire star system and is on course for one of the most populated sectors of federation space

I recall the Planet Killer from my father's old VHS taped episodes of TOS. It was also one of the more annoying missions in my PS2 game, Star Trek Shattered Universe, but at least I got myself a Taj Class Bird of Prey(Klingon Fighter) out of it, my favorite after the Terran Empire's Type III Phantom class fighter. The BoP fighter was the first to offer guided photon torpedoes, while the Phantom III had some badass MIRV photon torpedoes.
