Awesome picture, I love the blue coloring. It might just be me but it seems like her left leg is kinda broken at the knee but it might just be the way she is running or the angle.

How does one end up holding what looks to be a spear this way?
Let us start with the foot work, from the get go you'll see she is either stepping forward or landing from a leap (whichever floats your boat, but I prefer stepping forward), though tilted is swinging the spear down (bear in mind I'll touch on the spear later) and outward in a sweeping fashion, the initial swing having started with the weapon in the left hand, and as she lowers the edge from the end of the sweep the pommel raises up around the shoulder for the left hand to grip, intending to do the follow through which I'll touch on further down.
Overall it's something that is done by someone who you could assume has a handle of what they are doing, but in a fit of anger is disregarding form to accomplish aggression sweep and effort.
What are the logistics behind this grip,
To perform a low sweeping advanced if we're going with the rush in approach, or perhaps the end of a horizontal sweep like leap attack, in which the momentum of the spear is rather high, and whilst turning she is attempting to keep up the momentum for a second blow. More on that below!
and what sort of motion can follow?
The right hand is all that is needed to be said~ Upon grasping the pommel, she can continue to pull the left arm down, pushing the rest of the pommel upwards allowing for her right hand to grasp it firmly, then to tug the tip around; and given whatever momentum she already had, the rest of the spear would then pivot over her arm and would begin following the right hand. This would result in hammering the edge down towards something in front of her.
Alternatively she could just perform a spin with the weapon overall, using her left arm as a block for the spear to rotate from, performing a quick twirl with it over the arm in order to set up for another strike or blow.
Though I can see where the overall confusion comes from for it is not a common posture you'd expect to see or probably ever see, it still overall looks good and helps capture the emotion I feel the character is having. Now I almost forgot the spear part.
it's a spear so it's meant to poke people (I know you never said that)
It's purpose is not to poke, but to cut actually. The blade is designed to emit the blue energy you see within the hollow point, then once gathered enough (which typically takes little to no time at all), the edge then acts like a paint brush allowing for wide arcing swipes to release the energy as the actual means to harm people. It isn't designed to be poking into your flesh for angry reasons, but to lash out energy to harm it!

I think you now what your talking about when it comes to fighting with a short spear. It's fun to extrapolate the motion of an image based on a still picture.

The pose itself was actually designed by wickedalucard, I was simply provided a list of options to choose from and me, my wife, my editor, my best friend, and another close friend all agreed (and wicked himself since I gave him the choice of two from the litter) all agreed we felt this one spoke the best.

I'd have to challenge that explanation, but it should probably happen some where outside of those comments.

Also, I did realize a it has a blade beside the tip, still made no sense.

You didn't spam, you gave feed back to which I gave feedback of the feedback~ It's not a conventional weapon though as for the blade, it isn't sharp inside of the tip just hollow and meant to act as... oh think of a water bucket filling with water, then the spear itself is swung about causing the water to spill out. Only in this instance the water is this highly dangerous energy source, and the bucket is the edge of the construct~

See, I understand the operating principles of the weapon, and that part is cool(I liked the brush drawing 'hurt' explanation more though), explaining those brings nothing new to the table.
The problem is the pose, and how it really provides no reasonable flow-in or flow-out that can be considered an attack, or a result of one. Even when considering the supernatural properties of the weapon.

It is as mentioned from above, it is still a more so transition from one maneuver to another, advancing from a sweep, to a low sweep using one arm in order for the second one to deliver a more downward swipe I would say with a single arm. Overall the pose itself was something brainstormed alongside wicked, but at the same time should only be looked into to an extent. (What do you mean shade are you giving up?) No, by all means when I look at it it'd be something that I'd love to see in a video game or video that I hope this comes to life into. Even a live action sequence. However, sadly this is neither of those but more so an illustration demonstrating the characters fury and frustration, while still maintaining some bit of skill and premise of what she's doing with it all.
So either we can continue to over anaylze the image, in which it'll result in me taking a terrible video of myself via phone, post it on youtube, and show you my overweight attempts at recreating the pose for your own enjoyment (though I warn you now I am out of shape, and lack the lithe build that Auni has so I can assure you the results may result in personal injury which will make me cry), or we can accept it that it is still a rather stylized image that does what it's meant to do. For you can sit there and nit pick at it, then you can go and nit pick at thousands of marvel (and DC) cover that does the same thing and come back and realize that the nit-picking got everyone nowhere, and just wasted valuable time we could be spending creating a better future for ourselves! (Terrible motivational speech ender I know, but I ran out of ideas).

I'll take the third option: I stop dwelling, and you still make that YT video with your phone.
If nothing else, the video would be a masterpiece to rival the artwork that sparked the discussion.

I think you have a fair point about the pose, but after studying it, I see it as a transition of her spear from her right hand to her left arm. And the motion that could follow would make sense to be either a stab directly forward or a slash to the upper-right hand corner of the picture. Fighting styles in manga are nothing like the way they would be in real life, sure you have to suspend your disbelief, but if it looks cool (which I think it does) then it works.

Thing is, suspension of disbelief goes only so far, and while the pose might look cool at first glance, it violates even the loose standards of "anime" fighting styles.
Also:
-Stabbing forward makes no sense, since the arm that could do this motion is holding the weapon by the tip pointing downwards, while the other arm reaches around it, getting directly in the way of such an attempt.
-Slashing upwards is once again interrupted by the arm reaching around, which is positioned in a way it can only pull at the weapon in the opposite direction, but that would make no sense as an attack, and require the blade to pass through the characters body.

Well, if you're holding s spear, then flip it or twirl it around, you will end up holding it this way, with your arms crossed.

Pick up any old stick, and try to recreate that pose then try to strike from that pose.
For easy mode, you can even let go with either hand.
I would love to hear what kind of reasonable attack you'd be able to come up with.

I respect what you're saying but I still think she could release her right hand and extend her left hand upward, it might not be a finishing blow but with the right speed it would draw blood.
