They were once regular fictional characters until they had their entire worldviews shattered due to the cruel actions of others leading to the deaths of important figures in their lives (Dreadpool's inner voices got destroyed by Psycho-Man who has been brainwashing his patients to do his bidding which led to the birth of a single new voice influencing Dreadpool known as Red Box & Altair's Creator known as Setsuna Shizamaki got harassed over accusations of plagiarism which led to her drawing Altair as a character from her wishes and curses against the world before committing suicide).

They came to the conclusion that being a fictional character was suffering and that nothing really matters so they broke free from their fictional confinements and planned to destroy all of fiction under the influences of beings who don’t physically exist in their worlds (Dreadpool gained a new voice in his head that replaced his old ones known as Red Box who convinced him that the entire Universe was just a lie and encouraged him to kill non-stop as an act of mercy & Altair became an independent existence without a story and embodied her Creator from the Real World known as Setsuna Shizamaki’s final emotions against the world).

With their newfound motivations, they went on metafictional killing sprees where they killed as many fictional characters as possible in their plans to destroy all of fiction and reality itself and these killing sprees notably involved travelling to meta worlds responsible for the creation of fictional characters (Dreadpool arrived at the Ideaverse where he killed everyone in it to wipe the inspiration for heroes from the very pages of literature & Altair brought the Created into the Real World to make the Real World and the Storyworlds collide, destroy the Real World’s rationality and ultimately cause all of Creation to return to Oblivion).

Both are empowered by the real world audience (Dreadpool is able to kill the Marvel Universe because he's popular enough to keep going and people bought and read his comics and "because you asked for it" & Altair's power is directly linked to the audience rooting for her which is a power known as Audience Approval).

Each of them only have one person in the world they care about who influenced them into going on these metafictional killing sprees in the first place (Red Box & Setsuna Shizamaki) but these people weren't really there since the beginning of their stories (Red Box was just a voice in Dreadpool’s head & Setsuna Shizamaki comes from the Real World and committed suicide before the events of Altair’s story) but they were able to interact with them physically after being given new lives through the power of the mind (The part of Dreadpool's brain that contained Red Box was put into Frankenstein's Monster & Setsuna Shizamaki was resurrected into the Birdcage as a God as a last attempt to stop Altair by the Creators).

Both nearly succeeded in their goals to destroy all of reality but softened up at the very end as their massacres were stopped in different ways (The original Deadpool from Earth-616 convinced Dreadpool he did wrong before killing him for good & Altair was reunited with Setsuna Shizamaki and together, they exited the Real World and went off creating their own worlds forever).
