I really love this photo! The concept, the models, everything!
The one suggestion I would have is overall there is some mild lacking in depth, there is a lot going on in the background with the lack of dynamic lighting and I can't seem to focus on the couple and the billiards table as much as I would really like to because this shot is well done! I would just say find a way to bring more color and life to the scene at hand and force the mood lighting on them.
STILL! Better than anything I could do! (ok that wasn't saying much), and this is definitely a unique and interesting concept that most people don't venture into, especially with the controversy that sparked in the comments which is completely irrelevant to the beauty of the picture and your talent as a photographer! I'm sure everyone develops a unique and different opinion about the story behind it and that's what makes it so fascinating!
I love this, more than I can possibly say! If my critique bothers you, feel free to ignore it because I still simply adore this! ♥

Your critique doesn't bother me at all! I'm always eager to better myself and my work! And I agree with the lack of DOF, but that is the handicap of f2.8 in a wide-angle setting... Perhaps a f1.2 could do the trick, but I don't have that lens. Thanks for your comment! Much appreciated!

Ah understandable, and even in post-editing it's hard to get the effect without it looking cheesy. And lenses are super expensive!

She's not even smoking. It's photoshopped in. The artist also said much earlier that it's quite hard t get a woman in their country anywhere near a lit cigarette, much less smoke one. Personally, seeing as how they also look very unhappy, I think the point of the photo is that that kind of Life style isn't healthy, especially smoking while pregnant.

I knew when I saw this DD pop up in the bottom of my page all the internet trolls would come out in full force, come back a few hours later and it's a feast of squabble.
I like how this has caused such an uproar, it makes me laugh.
