... I was rummaging through my hard drive and I found this from Arkham Asylym... basically just a line-up trying on different 'tops' on Joker.. at one point, the idea of 'Doctor Joker' was more prevalent, hence the 'butcher apron' and the Doctor's coat.Related content

The fist launcher got me going.
I'm thinking inside the fist is filled with a special powder. Outside the fist is covered with holes. Once launched by striking the victim, it releases the powder within the impact and makes the victim convulse with a seizure-type shake and drops down the floor without no movement, which is paralysis.
