The picture itself was made by me and is just fanart, just like all my other pictures but the patch of the D.S.O is a scan of an original patch that was released with the strategy guide for RE6.

I haven't seen it in the game itself, but otherwise the D.S.O-logo is an existing design like the logo of the BSAA.

The same, although the patch of Jake was visible in the game. Capcom released a set of the patches (DSO, Spider, BSAA) with the english strategy guide and various versions with different resolutions can be found on the net.

Quick question how did you fix Helens's black arms?

When you open her in Blender you see a meshpart on her wrist standing out. I removed that, then joined the meshes of her hands and her arms and sewed the gap together by merging the vertices.
