Talos IV is the fourth planet in the Talos system, and the homeworld of the Talosian species. Not much else needs to be said that hasn't already been said.
The Enterprise was originally from Drex Files. And yes, I know that I used the film version of the spacecraft rather than the series version. This is because, while I agree that Matt Jeffries' version of the spacecraft's design was quite innovative and certainly a classic, I still tend to have a slight preference for Andrew Probert's version of the design.

So who's to say that at some point between TMP and TWOK, they didn't pay a return visit?

Well, for one, doing so would've carried the death penalty.

That wouldn't have stopped them if there was a real, emergency need. The Enterprise crew has been known to ignore rules and regulations if they have to in order to get the job done.
