ver 0.82 EDIT NOTE:1. Added every names of the Prothean Imperial Territory Empyreans
2. Added sector borders
3. Put research facilities in a separated category because there are too many primitives.

I think this was place of very last battle between Protheans and Reapers.
Behind Omega 4 Relay. This was ideal place for last sanctuary, even Reapers couldn't fight supernovas and blackholes around.
We know that Protheans could duplicate mass relays. And Omega station as mining facility for material to build everything necessary is very logic

Protheans couldn't duplicate a full-scale mass relay. They did however managed to build a miniature one (or two, if you count the Relay Monument on the Citadel as well).
IMO it's a major plot hole for the Relay Monument to be a one-way conduit installed by the Prothean though. Because it's the Keepers' duty to maintain the Citadel for the Reapers, and thus they should have removed it once it was installed.... But anyway, I'll disregard the plot hole and pretend it doesn't exist because it'd burn my brain out to fill that in.
My theory was that the Reapers built the Omega-4 relay after they wiped out every space-faring species and stationed the converted protheans (Collectors) off into the galactic center....

Or Omega 4 was another trap, allowing desperates to see unique moddable relay to the safe sanctuary in the Core, and, in the end, simply slaughtered by the Reapers.

Emotional patterns there. We searched for the lesser ones. Help them ascend, embrace the evolution. Some of us disagreed. Makers. Constructors. War, was there.
Dark Ones won. Betrayal.
Who are these 'Dark One's'? How can we stop them?
There are always three. No more. Embodied in Constructors. Their creations. Leading them. Nazara. Deception. Krell. Nightfall. Lantae. Death.
They will never stop. Their wisdom is wrong. Knowledge corrupted. We lost. Never forget. Never forgive.

Now this sounds like what was rumored to be in the Extended Cut.
Honestly I was tempted to do a Darksiders-Mass Effect cross-ver art piece when I head about the rumor. Like having sci-fi angels fighting over the Reapers.
Maybe you should write up a story based on this? It looks fairly workable.

Too much school, then whole Fest of Fantasy (biggest meeting of sci-fans in my country), then work...not much time to left for fanfiction in english 
Basically, I think some species ascended to some deities, and they created Constructors as invisible armada of their wisdom and power for real space. Just group of them. First of the 'Reapers' were intelligent, but harmless species, living in spheres of some gas giant, no knowledge or materials to see a space behind clouds.
'Makers' convince them to let their bodies to improve and new holy purpose. Every Constructor, even as cyborg, had freedom of choice, though. Some of them decided to be alone, rest went with their saviors, embodied in best of them. Nazara was the first one.
In these time, indoctrination was simply defense technology, if some species looked aggressive, this tech appease them, pacified them, change their way of thinking, but not a slaves.
One day in the new galaxy, they saw how a few species with their knowledge destroyed another one's. How created machines to destroy them. Choose war instead research and evacution even in times, when homeworld's sun going to explode in supernova.
And in next eons, they saw similar pictures. So some of the Makers were convinced, that organics, the lessers, are doomed anyway: idea of Cycles was created right now, by the Dark One's, as called them others gods.
There were three fractions, Dark One's, Heralds and Lightbringers. Heralds were loyal to original purpose. Their holy mission is peaceful, and consequences are up to others, what they did with their help. In the end, they were disgusted by the Dark One's, that they dissapeared in the dark space forever, to new galaxies.
But for Lighbringers, it werent enough. For them, Dark One's were a traitors and pure evilness, trying enslave the lessers for their wrong purposes. Especially, when Lighbringer gods knew, that they created virus to take freedom from minds of their followers and repurposed indoctrination for slavery of the lessers.
Lightbringers hadnt enough allies to destroy the Dark One's at once, but at least they begun apocalyptic war with creating black holes and exploding star systems to destroy always at least one of the traitors.
They almost won, but one god in constructor body betrayed them - Krell. Harbinger.
And the last Lightbringer in crippled body, fallen from sky promised vengeance on him, and other traitors. One day, his automatic systems repair him and allow him to fly up to surface and space. Darkness must be destroyed. No matter how, or when.

I wonder what would happen if two galaxies collides.
Like if there are a group of reapers being assigned to manage a galaxy, does two galaxies collided into one makes half of the reapers gone layoff?

Well, ancient Reapers should looked different. And only a few of them were mechanical 'gods' from merge with 'Makers' - rest of them were normal minds of that species of intelligent giants from gas planet.
These 'Gods' what helped species of giants reach the stars and be Constructors, had just privilege for be prophets and leaders - this is reason why Sovereign/Nazara could do things for other Reapers impossible, and why Harbinger is supreme leader despite how every Reaper is intelligent (except fact, that Dark One's enslaved them secretly through own virus, so they couldnt leave them, as fraction of 'Heralds' did - but they dont know that).
Harbinger: Ah, so you've joined us now, in the end of another cycle? Did you see the truth at last?
Constructor 451: I see only light! All traitors must fall!
Harbinger: Come to me, you pathetic relic of lost times. You're already dead. Forgotten. Lost number, without name. We are eternal.
Constructor 451: We would never leave our calm and peaceful gas giant, if we knew, what became from you all! Disgusting creature of darkness! Die!
Harbinger: Yes. Come to me. We demand your end. We will destroy you with all of these organics.

Or what would happen, if soul of the organic would ascend as well, and combine yourself with the Reaper?
